This chart can be found in the Forbidden Woods. It is in a chest in the northernmost room on the basement floor.

This chart can be obtained after the girls from the Forsaken forest are rescued. If you give the father of the now-rich girl Maggie 20 Skull Necklaces, he will reward you with the chart.

This Chart is in a chest on a small island near Forest Haven. It can be reached by using the Deku leaf when jumping from the top of the main island. You jump from the exit that leads to the Forbidden woods when the wind is blowing to the south-east. The chest is on the southernmost small island.

This chart can be purchased from Beedle. He sells it on his shop ship near Rock Spire Isle (Sector C2) for 900 rupees.

This chart can be found in the Wind Temple. It is hidden in a chest in the room next to the central room with the giant fan on B1 level. The chest will appear if you defeat every enemy in the room.

This chart can be found in the Dragon Roost Cavern. It lies hidden in a chest in the room to the left of the big central lava room. You have to cross a lava chasm with the Grapple hook to get it.

This chart can be found in the Earth Temple. It lies hidden in a chest in the room to the left of the big mirror puzzle room on the B2 floor, which you have to complete to gain the boss room key. The chest will appear after you beat the Stalfos emerging from the coffins.

This chart can be obtained on the Two-Eye Reef (G4) The chart is in a chest that will appear on the reef after you destroy all battleships and cannons that guard the reef. 
Reward: Secret Cave Chart

This chart can be obtained in the submarine near Headstone Island (G3) It is hidden in a chest that will appear after all the rats on board are defeated. 
Reward: 200 Rupees

This chart can be found in the Forbidden Woods. It is hidden in a chest in the second room of the dungeon on 3F level. The chest has boomerang plants on it, which have to be removed to open it.

This chart can be obtained on the observation platforms near the Seven Star Isles. It is hidden in a chest that will appear after you defeat the Wizzrobes roaming around the towers.

This chart can be obtained on Spectacle Island (B3). It is the prize for completing the Barrel-Bombing mini game twice.

This chart can be won in the Windfall Auction. The auction is held at night on the ground floor of the rich house.

This chart can be obtained on the Four-Eye Reef (B1) It is hidden in a chest that will appear when you clear the reef of battleships and cannons. 
Reward: Island Hearts Chart

This chart can be found in the Earth Temple. It is hidden in a chest in the room where you get the small key on B1 level. The chest will appear when you defeat all the enemies in the room.
Reward: Piece of Heart

This Chart can be found on the Cyclops Reef (Efficient, right?) Just as with the other reefs, the chest containing the chart will appear once you destroy all the cannons and battleships near the reef. 
Reward: Light Ring Chart

This chart can be found in a submarine near Northern Fairy Island (A3). 
Reward: 200 Rupees

This chart can be obtained by winning ‘ Zee Fleet Game’ on Windfall Island. You have to do it using 19 or less cannonballs this time.

This chart can be obtained by showing the gossiping ladies on Windfall Island a color pictograph of Lenzo talking to Minenco.

This Chart can be found on the Cliff Plateau Isles (F7) It is in a chest on the highest cliff. The cliff can be reached through the underground passage. A dying tree is there as well.

This chart can be found on the Six-Eye Reef (D4) The chest will appear after wiping out the battleships and cannons.

This chart can be found on the Private Oasis (E5)  It is in a chest on the upper part of the island near the top of the waterfall. You can reach it with the Hookshot.

This chart can be obtained on Horseshoe Island (G1) If you blow all the Boko Nuts into the holes, you will be able to reach the little platform in the center of the island. On it is a chest containing the chart.

This chart can be found on Windfall Island. If you enter Lenzo’s shop from the balcony (Reachable by Deku Leaf from the ferris wheel) You will find yourself in a secret room with a chest containing the chart.
